Backdoors are server-side malicious scripts which are intended to perpetrate malicious acccess to the server. db_shell is special kind of web-shell aimed on the work with the database. Using such shell, the attacker can view and modify victim's database from the browser. It is usually used for injecting another kind of backdoor to the DB for later use.
